AM I ELIGIBLE?

Tamworth Meals on Wheels are available to those people funded by the Australian Government under the Commonwealth Home Support Program (CHSP) and eligibility is determined by My Aged Care national entry point.

Meals on Wheels are available to carers of older people or those with a disability. The Carer Gateway is a new national website and phone number to help carers access information and advice and connect with services in their local area. It is the first stage of the Australian Government’s Integrated Plan for Carer Support Services. Carers can use the Carer Gateway online at www.carergateway.gov.au or calling 1800 422 737 (Mon to Frid, 8am to 6pm).

Meals on Wheels are also available to people with a disability and those who are not eligible for funding assistance, but who wish to purchase meals on a full cost basis.

HOW DO I REGISTER?
If you are 65 years or over (or 50 years or over for Aboriginal and Torres Strait Islanders), we require a referral from the government organisation, My Aged Care. To be referred to Tamworth Meals on Wheels, please call My Aged Care on 1800 200 422 or visit the My Aged Care website by clicking here. Alternatively, you can complete the form below or contact our friendly staff on 02 6765 8999, and we can simply arrange the referral on your behalf. The process typically takes up to 48 hours, however, we can commence our service to you in the meantime if needed.

If you are under the age of 65 (or under 50 years for Aboriginal and Torres Strait Islanders) and living with – or caring for someone with – a registered disability, we generally require a referral from the NDIS.
